Everyone is asked to complete this form, which is different from any school or PSR forms and needs to be filled in for every child. You may return the completed form as hard copy to Sr. Yvonne, or by email to sryvonne@holyfamparma.org

There is a Sacramental Fee of $25.00. PSR families have already paid this. The families from HFS and IWA should pay when they send their registration form, (December 6th).
