Please find the attached forms for the Sacrament of Confirmation. Please be sure to accurately complete all sections. Please return both Registration and Sponsor Certificate in hard copy form, NOT digitally. Forms are to be returned to Sister Yvonne’s office as soon as possible, but not later than Tuesday, February 28, 2023.
